May 6, 2021 Opinion or Order Filing 3 ORDER: The plaintiff MUST either (1) pay the full $402.00 filing fee to the Clerk of Court or (2) submit an application to proceed as a pauper. 28 U.S.C. 1914, 1915(a). Because the plaintiff appears to be a prisoner, any application to proceed as a pauper MUST include an inmate trust account statement (or institutional equivalent) for the 6-month period immediately preceding the filing of the complaint that is obtained from and signed by the custodian of the plaintiff's trust account. The Clerk is DIRECTED to mail the plaintiff a blank in forma pauperis application for prisoners (short form). Second, the plaintiff MUST clarify in writing his current mailing address. See M.D. Tenn. L.R. 41.01(b). The plaintiff's responses concerning (1) the filing fee and (2) his correct mailing address MUST be received by the court within 30 DAYS of the date this order is entered on the docket.The Clerk SHALL mail this order to both addresses provided by the plaintiff (the residential address in Jacksboro, TN, and the Claiborne County Jail address in New Tazewell, TN).
