February 23, 2022 Opinion or Order Filing 6 ORDER: William Creasy, an inmate at the Trousdale Turner Correctional Complex in Hartsville, Tennessee, has filed a pro se Petition for Writ of Habeas Corpus under 28 U.S.C. 2254 (Doc. No. #1 ) and has paid the filing fee. However, while the seven-page Petition utilizes the first two pages of the appropriate form for seeking relief under Section 2254, its remaining five pages are handwritten and do not include all the information pertinent to the initial review required by Rule 4 of the Rules Governing Section 2254 Proceedings. Accordingly, the Court will require Petitioner to file an amended Petition. The Clerk of Court is DIRECTED to mail Petitioner a blank form for filing a Petition for Writ of Habeas Corpus Under 28 U.S.C. 2254 (Form AO 241), along with a copy of his original Petition (Doc. No. #1 ) for his reference. Petitioner's amended Petition MUST be filed on the provided form within 30 DAYS of the entry of this Order and must provide responsive information in all fields of the form. Petitioner is cautioned that failure to comply with this Order, or to request an extension of the deadline before its expiration, may result in the dismissal of this action.
