ORDER: To file a civil lawsuit, a plaintiff must submit the civil filing fee or an application to proceed in forma pauperis. In addition, if seeking pauper status, a plaintiff must provide a certified copy of his inmate trust fund account statement for the 6-month period immediately preceding the filing of his complaint as required by 28 U.S.C. 1915(a)(2). Plaintiff Roby has submitted an application to proceed in forma pauperis. (Doc. No. #2 ). However, he has not included a certified copy of his inmate account statement for the 6-month period immediately preceding the filing of his complaint as required by 28 U.S.C. 1915(a)(2). Plaintiff Roby is DIRECTED to submit this missing item within 30 days of the date he receives this Order if he wishes to proceed as a pauper. Plaintiffs Doust and Page have not submitted their portion of the civil filing fee or an application to proceed in forma pauperis. Therefore, the Clerk is DIRECTED to send Plaintiffs Doust and Page each a blank application to proceed in forma pauperis for prisoners. Plaintiffs Doust and Page are DIRECTED to complete an application to proceed in forma pauperis and return the properly completed application to the district court if they wish to proceed as paupers.
