ORDER: Charles Walker, an inmate of the Rutherford County Adult Detention Center, filed a pro se civil rights complaint under 42 U.S.C. 1983 (Doc. No. #1 ) and submitted an in forma pauperis application #2 . Here, Walker submitted an unsigned first page of the Court's form affidavit and omitted the certified trust account statement. The Court will therefore allow Plaintiff to submit an amended application within 30 DAYS of the date this Order is entered on the docket. Should Plaintiff fail to comply with this Order or seek an extension of time by the deadline, this action may be dismissed for want of prosecution.
