Within 30 DAYS of the date this Order is entered on the docket, White MUST pay the full $405 civil filing fee to the Clerk of Court or submit an application to proceed in forma pauperis. Failure to comply or request an extension by the deadline will result in dismissal. Fed. R. Civ. P. 41(b). The Clerk SHALL mail Whyte a blank long-form in forma pauperis application.
