April 30, 2024 Filing 7 MEMORANDUM OPINION AND ORDER: To proceed as a pauper, Plaintiff MUST submit the required certified statement within 30 days of entry of this Order. The required certified statement need not be notarized. The facility custodian of Plaintiff's inmate account should simply sign and date the inmate trust fund statement (which makes the statement "certified"). Plaintiff cannot certify his own statement. If Plaintiff submits a timely amended complaint and the required certified statement of his inmate trust fund account activity, the court will screen the amended complaint pursuant to the PLRA upon receipt. March 18, 2024 Filing 4 ORDER: Plaintiff has not submitted the civil filing fee or an application for leave to proceed in forma pauperis along with the certified statement. He must submit one or the other for this action to proceed. For Plaintiff's convenience, the Clerk is DIRECTED to mail the plaintiff an Application for Leave to Proceed in District Court Without Prepaying Fees or Costs for prisoners (Short Form, AO 240) (often called an IFP application). Plaintiff must complete and return this application within 30 days of entry of this Order if he wishes to seek pauper status. In addition, Plaintiff must submit a certified copy of his inmate trust fund account statement as required by 28 U.S.C. 1915(a)(2).
