However, his statement is not certified as required by statute. (See id. at PageID 27). Thus, to proceed as a pauper, Plaintiff must submit the required certified statement within 30 days of the date of entry of this Order. Alternatively, Plaintiff may submit the full civil filing fee of $405 by the same deadline. Plaintiff may request an extension of time to comply with the Court's instructions if he requests an extension in writing within 30 days of the date of entry of this Order.
