Filing 3 ORDER Plaintiff/inmate, Christopher Wayne Robertson, appearing pro se, submits a civil case. He applies to proceed without prepaying the filing fee (ECF 1). However, Plaintiff has not as required by statute submitted (1) a proper form affidavit that includes a statement of all assets such prisoner possesses that the person is unable to pay such fees or give security therefor; and (2) "a certified copy of the trust fund account statement (or institutional equivalent) for the prisoner for the 6-month period immediately preceding the filing of the complaint. obtained from the appropriate official of each prison at which the prisoner is or was confined." 28 U.S.C.S. 1915(a) (2022) (emphasis added.) Instead, the in forma pauperis application Plaintiff provided was handwritten and incomplete (ECF 1).IT IS HEREBY ORDERED that Plaintiff shall within thirty days file with the Court the completed & signed required affidavit (titled, Application to Proceed in District Court Without Prepaying Fees or Costs, sent with this Order) and certified copy of Plaintiffs inmate trust fund account statement(s). If Plaintiff was held at more than one institution during the past six months, Plaintiff shall file certified trust fund account statements (or institutional equivalent) from the appropriate official at each institution where Plaintiff was confined. The certified trustfund account statement(s) must show deposits and average balances for each month. If Plaintiff does not fully comply, this complaint will be dismissed.
