Filing 6 DOCKET TEXT ORDER. Plaintiff is proceeding in forma pauperis. ECF No. #5 . Accordingly, the court must "issue and serve all process." 28 U.S.C. 1915(d). If Plaintiff intends to have the court effectuate service, counsel must complete and return to the Clerk of Court a summons and Service of Process form for each Defendant. DUCivR 3-2(c)(2)(A)(i). Those forms are available on the court's website at: https://www.utd.uscourts.gov/usdc-forms. Upon receipt, the Clerk of Court must issue the summonses and send a service packet for each Defendant to the United States Marshals Service for service of process. If Plaintiff elects to effectuate service without court assistance, counsel may submit summonses to the Clerk of Court without a Service of Process form. Upon receipt, the Clerk of Court must issue the summonses. Plaintiff must then properly serve the Defendants or move for an extension within 90 days from the date the complaint was filed, as required by Fed. R. Civ. P. 4(m).
