ORDER granting #4 Motion for Leave to Proceed in forma pauperis Under 28 U.S.C. 1915 and DUCivR 3-2(b), the court must screen the complaint. Plaintiff is proceeding in forma pauperis and must provide the court with the names and addresses of each named Defendant so the court can effectuate service. See 28 U.S.C. 1915(d) (The officers of the court shall issue and serve all process and perform all duties in [in forma pauperis] cases.). Plaintiff is ordered to complete and return to the clerks office a summons (form AO 440) and Service of Process (form USM 285) for each defendant.
