ORDER granting #2 Motion for Leave to Proceed in forma pauperis. Pursuant to 28 U.S.C. 1915(d), the court will issue and serve all process. If Plaintiff intends to have the court effectuate service, Plaintiff or counsel mustcomplete and return to the Clerks Office, via hand-delivery, mail or email, a summons (form AO 440) and Service of Process form (form USM 285) for each defendant, see DUCivR 3-2(c)(2)(A)(i), available on the courts website: https://www.utd.uscourts.gov/usdc-forms by July 23, 2026. If certain Defendants in this action are identified as federal employees or ifPlaintiff has alleged claims against a federal agency, then Plaintiff must also complete and return to the clerks office a summons and form USM 285 to be served on the Civil Process Clerk at the U.S. Attorneys Office for the District of Utah.Upon receipt, the Clerks Office must issue the summonses and send a servicepack for each Defendant to the United States Marshals Service for service of process.If Plaintiff elects to pursue service without court assistance, counsel may submit summonses to the Clerks Office without a Service of Process form. Upon receipt, the Clerks Office must issue the summonses. Plaintiff must properly serve the Defendants or move for an extension within ninety days from the date the complaint was filed, as required by Fed. R. Civ. P. 4(m).
