Filing 33 ORDER that Petitioner must pay the filing fee of $5.00 or submit a completed application for leave to proceed in forma pauperis , including a certified copy of his prison trust account statement on or before 9/16/11. Mr. Williams must su bmit an amended petition for writ of habeas corpus on the appropriate form by 9/16/11. If Plaintiff seeks to file a separate 42 U.S.C. § 1983 complaint, he should submit the complaint, on the Court's form, along with a properly completed ap plication to proceed in forma pauperis.
