July 26, 2006	Filing 4 MEMORANDUM OPINION & ORDER; 1) sole named dft is Denise Black, in her individual capacity; 2) clerk forward by cert mail, one copy of complaint, and instant order to Offc. of Gen Cnsl for KY Dept of Corrections, Frankfort, KY; Gen Cnsl shall have 10 dys to file a notice of waiver of service against any named dft; if waiver is not filed w/in said 10 dys clerk shall notify the PSOffc 3) the answer to the complaint shall be filed no later than 60 dys after notice of waiver is filed; if service is r equired the answer shall be filed no later than 20 dys after service of summons 4) pla shall keep clerk informed of mailing address; failure to notify any address change may result in dismissal 5) for every further pldg or other document he wishes to submit for consideration the pla shall serve upon each dft or counsel a copy of pldg or other document; the pla shall send the original papers to be filed w/clerk w/a certificate stating the date a true and correct copy of document was mailed to eac h dft or counsel ; if Judge or Mag. Judge receives any document which has not been filed w/clerk or which has been filed but fails to include the certificate of service of copies the document will be disregarded by court.
