MEMORANDUM OPINION & ORDER; 1) dfts are LSCC Denise Black, Dr. Ronald Fleming, Keith Helton, and David Green, construed as being sued in their offc and ind. capacities 2) clerk is directed to forward by cert mail, return receipt requested, one copy o f comp and MO&O herein to the Offc of General Cnsl for KY Dept of Corrections, Frankfort, Kentucky, General Counsel shall have 10 dys from entry of this order to complete and file a notice of waiver of service against any or all named dfts; if a waiv er is not filed w/in 10 dys clerk shall notify PSOffc 3) the answer or other resp to the complaint shall be filed no later than 60 dys after the notice of waiver of service is filed; however if service is required the answer shall be filed no later t han 20 dys after service of summons 4) pla shall keep clerk informed of mailing address; failure to notify clerk of any address change may result in dismissal of this case 5) for every further pldg or document pla shall serve upon each dft or counsel a copy of pldg or other document; pla shall send orig papers to be filed w/clerk w/certificate stating the date a true and correct copy of document was mailed to dft or counsel; if Judge or Mag.
