Belen, Eddie Montgomery, Kentucky Department of Corrections are DISMISSED as parties to this proceeding. 2) Deputy Clerk shall prepare a "Service Packet" for Dfts Capt. Lawrence Betts, Lt. Joel Helmburg, Sandy Coyler of Aramark Food Service, L.P.N. Paula Auxier, and Correct Care Soltuions. Include, A) summons form, B) amended complaint, C) Order granting Pla IFP status, D) this Order, E) a completed USM Form 285. Lexington Deputy Clerk shall send the Service Packets to the USMS in LExington and enter delivery date into the record. 3) USMS shall personally serve a Service Packet upon each dfts at address provided by Pla. 4) Kilgore must advise Clerk of any change of his current mailing address. 5) If pla wishes to seek relief from the Court, do so by filing a formal motion to Clerk's office.
