IT IS FURTHER ORDERED that the Clerk shall issue process or cause process to be issued upon the complaint. IT IS FU RTHER ORDERED that plaintiff shall pay an initial filing fee of $12.00 within thirty (30) days of the date of this Order. Plaintiff is instructed to make his remittance payable to Clerk, United States District Court, and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittance is for an original proceeding. FURTHER ORDERED that the Clerk shall mail plaintiff a copy of the Courts form motion to proceed in forma pauperis - prisoner cases. FURTHER ORDERED that if plaintiff fails to comply w ith thisMemorandum and Order, the Court may dismiss this action without prejudice. Response to Court due by 8/17/2012. Signed by Honorable Henry E.
