Filing 6 MEMORANDUM ORDER (Ordering Plaintiff to Return Forms) The Court has already received the documentation relating to his trust account. Therefore, Plaintiff does not need to obtain this information in order to comply with the Courts order. Rather, Plaintiff must complete and return an in forma pauperis affidavit and a consent to collection of fees form if he wishes to proceed with this action. Accordingly, it is ORDERED that: 1. Within twenty (20) days from the date of entry hereof, Plaintiff must submit a statement under oath or under penalty of perjury that: (A)Identifies the nature of the action; (B) States his belief that he is entitled to relief; (C) Avers that he is unable to prepay fees or give security therefor; and, (D) Includes a statement of the assets he possesses.
