Filing 4 ORDER: it is ORDERED that: 1) On or before 1/22/2021, Plf shall submit an inmate account statement reflecting the average monthly balance in his prison account and the average monthly deposits to his account for the 6-month period immediately preceding the filing of the instant complaint, as further set out in order; 2) The Clerk of Court SEND a copy of this order to the inmate account clerk at the Staton Correctional Facility; Plf shall immediately inform the court and Dfts or Dfts' counsel of record of any change in his address; Failure to provide a correct address to this court within 10 days following any change of address will result in the dismissal of this action.
