ORDER: it is hereby ORDERED that Plf must sign the fourth page of the Complaint and also sign second page of the AO Form 240 and then re-file the executed signature pages no later than 3/4/2024; Further, consistent with 28 USC 1915(b)(1), it is hereby ORDERED that on or before 3/4/2024, Plf must also submit an inmate account statement reflecting the average monthly balance in his account and the average monthly deposits to his account for the six-month period immediately preceding the filing of the Complaint on 2/7/2024; DIRECTING the Clerk to provide a copy of this Order to the inmate account clerk at the facility where Plf is incarcerated; DIRECTING the Clerk to mail the second page of the AO Form 240 (Doc. #2 at p. 2) and the fourth page of the Complaint (Doc. #1 at p. 4) to Plf and retain a copy for the court file; further DIRECTING the Clerk to furnish Plf with a pre-addressed, stamped envelope for use in returning the executed signature pages for filing, as further set out in order.
