ORDER: Although Plaintiff filed an #2 Application for Leave to Proceed in forma pauperis and a prison account statement with information as to the balance and deposits to such account from February of 2019 until January of 2020, he did not submit the requisite documentation from the inmate account clerk at Bullock showing his average monthly balance and average monthly deposits to his inmate account for the six-month period prior to filing this case. Thus, the documents filed by the plaintiff fail to provide the court with the information necessary for a determination of whether the plaintiff should be allowed to proceed in forma pauperis without prepayment of a filing fee in this cause of action. Accordingly, it is ORDERED that on or before September 1, 2020 the plaintiff shall file a prison account statement from the inmate account clerk at Bullock as further set out in the order. To aid the plaintiff in his compliance with this order, the Clerk is DIRECTED to provide a copy of this order to the inmate account clerk at Bullock. It is further ORDERED that the plaintiff shall immediately inform the court and the defendants of any change in his address. Signed by Honorable Judge Charles S. Coody on 8/12/2020.
