We note that appellant's IFP request is deficient in that it does not state the issues to be presented on appeal nor is a prison trust account statement included. A Form 4, financial affidavit, is enclosed to the appellant. Unless appellant provides this court with proof of paying the $505 filing fee to the district court or filing a compliant request for IFP status in the district court by September 28, 2023, this appeal may be dismissed for lack of prosecution.
