It is FURTHER ORDERED that Mr. Lounsbury shall obtain and utilize the appropriate court-approved form for either (1) filing a Prisoner Complaint and Prisoner's Motion for Leave to Proceed Pursuant to 28 U.S.C. 1915; or (2) an Application for a Writ of Habeas Corpus Pursuant to 28 U.S.C. 2241 and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 in a Habeas Corpus Action, along with any applicable instructions, from the Court's website at: www.cod.uscourts.gov, located in the "Quick Links" section of the Court's homepage under "Forms." He may pay the applicable filing fee in lieu of filing a 1915 motion and affidavit. It is FURTHER ORDERED that if Mr. Lounsbury fails to cure the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice.
