Filing 14 SECOND ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES. It is ORDERED that Plaintiff cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that Plaintiff files in response to this order must include the civil action number on this order. To be clear, in order to cure deficiencies, Plaintiff must submit a completed Prisoner Complaint and a completed Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 on the court-approved forms, with all required supporting documentation. These are the only appropriate filings at this time. If Plaintiff fails to cure all of the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice. The dismissal shall be without prejudice. The pending motions (ECF Nos. #5 , #7 , #10 , #11 ) are denied with leave to refile on the correct form with all required information, by Magistrate Judge Susan Prose on 11/8/2023.(ccuen, ) |
