October 3, 2022 Opinion or Order Filing 15 ORDER. Plaintiff has filed a fifty-seven-page complaint, including exhibits, but it appears incomplete. After review, it appears that Plaintiff's complaint is missing every other page. For example, Plaintiff's Statement of Case begins on a page numbered five, but it only the third page of the filing. See ECF No. 1 at 3. Additionally, Plaintiff indicates via an arrow that the statement continues onto the next page, but the following page begins by indicating Plaintiff's desire for a jury trial. Id. at 4. It appears that whatever caused this issue may also have prevented Plaintiff from indicating what relief he seeks in this action, as required by Federal Rule of Civil Procedure Rule 8(a)(3). Accordingly, Plaintiff is instructed to refile his complaint, correcting these issues, by November 2, 2022. Unless Plaintiff corrects these issues, it will be difficult for the Court to determine if Plaintiff is entitled to relief on his present claims, and the failure to correct the complaint could result in dismissal of the action without prejudice. September 2, 2022 Filing 7 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. 1915, permits him to do so if, among other things, he files an affidavit demonstrating that he is unable to pay the fee. To assist inmate plaintiffs in making this demonstration, the District of Connecticut provides them with a six-page form to be filled out. In this case, the plaintiff did not complete the entire form. Without a complete form, the Court cannot determine whether he is able to pay.The Clerk of the Court is respectfully directed to mail the plaintiff another copy of the application form at his address of record.
