Filing 8 ORDER: The Court has become aware that the Plaintiff is no longer housed at the MacDougall-Walker Correctional Institution as the Connecticut State Department of Correction website indicates he is now housed at the Corrigan Correctional Center. Therefore, it is not clear whether the Plaintiff received the Court's notice of insufficiency dated November 9, 2022. Accordingly, the clerk is instructed to send a copy of the Notice of Insufficiency [ECF No. 7] to the Plaintiff at the Corrigan Correctional Center. Failure to comply with the Notice of Insufficiency or this Order by December 21, 2022 may result in the dismissal of this action. Filing 7 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. 1915, permits him to do so if, among other things, he files an affidavit demonstrating that he is unable to pay the fee. To assist inmate plaintiffs in making this demonstration, the District of Connecticut provides them with a six-page form to be filled out. In this case, the plaintiff attempted his own facsimile of the form, but he omitted page four. Without that page, the court cannot confirm whether the plaintiff understands that, if he is granted in forma pauperis status, the filing fee will nevertheless be withdrawn from his inmate trust account in installments according to the schedule set forth in the Prison Litigation Reform Act.28 U.S.C. 1915 also requires the plaintiff to file a certified copy of his inmate trust account statement for the six month period preceding his complaint. In this case, the plaintiff did not file a complete certification. Specifically, the name of the inmate trust fund officer or prison counselor is missing. In addition, the plaintiff did not file an up-to-date certification. Specifically, the plaintiff certifies that he has a $0.00 account sum as of November 1, 2022, however, the available balance shown on the plaintiffs filed Prisoner Trust Fund Account Statement (ECF No. 3) shows a $300.00 deposit on November 3, 2022, resulting in a current available balance of $300.29.The plaintiff is directed to file a new, up-to-date application, along with a certified copy of his inmate trust account statement covering at least the period from April 30, 2022 to October 31, 2022. The Clerk of the Court is respectfully directed to mail the plaintiff another copy of the application form at his address of record. The plaintiff is directed to complete all pages of the form accurately and return it to the Clerk of the Court by November 29, 2022. If he does not do so, his case may be dismissed.
