ENDORSED ORDER. Plaintiff, Steven Warren, an inmate of the Florida penal system, filed a pro se unsigned #1 document in which he contends that mental health professionals at Florida State Prison do not help all inmates who claim to hear voices encouraging self-harm, but only help those whose families call "head" officials in Tallahassee. He claims he is hearing voices telling him to kill himself, and he swallowed a paperclip, which is causing him pain. Warren's filing is not a proper complaint, does not include all of the information required by the court-approved civil rights complaint form, and cannot commence a civil action. See Fed. R. Civ. P. 3 ("A civil action is commenced by filing a complaint with the court."); see also M.D. Fla. R. 6.04(a) ("A pro se person in custody must use the standard form[.]"). Accordingly, the Clerk of Court shall close this case and send Warren a civil rights complaint form. If Warren chooses to refile his claims, he may do so on the provided form.
