ORDER. The clerk shall send plaintiff a Prisoner Consent Form and Financial Certificate. Plaintiff shall have 30 days from the date of this order to correct the deficiencies in his in forma pauperis application by filing: (1) a prisoner consent form signed by an authorized prison official; and (2) a copy of his inmate account statement for the 6-month period preceding the filing of the complaint. Alternatively, plaintiff may pay the full $400.00 filing fee within 30 days. Plaintiff shall have 30 days from the date of this order to submit four service copies of the complaint (doc. #1 ).
