Filing 6 ORDER re #1 Complaint. The clerk shall send a section 1983 complaint form for use by prisoners to Plaintiff. This case number shall be written on the forms, and "Amended Complaint" shall be typed on the complaint form. Within thirty (30) days from the date of this order, Plaintiff shall file an amended complaint, compliant with the instructions set forth herein, and which shall be typed or clearly written and submitted on the provided form. Alternatively, and within the same time, Plaintiff may file a notice of voluntary dismissal if he no long wishes to proceed with this action. Failure to comply with this order as instructed may result in a recommendation that this case be dismissed for failure to prosecute or failure to comply with an order of the Court. Filing 5 ORDER GRANTING LEAVE TO PROCEED IN FORMA PAUPERIS. Plaintiff's first motion to proceed in forma pauperis (ECF Doc. #2 ) is DENIED as MOOT. Plaintiffs second motion to proceed in forma pauperis (ECF Doc. #3 ) is GRANTED, and Plaintiff will not be required to pay a partial initial filing fee under 28 U.S.C. 1915(b)(1). Pursuant to the Prison Litigation Reform Act of 1995, 28 U.S.C. 1915 (as amended), even though Plaintiff is granted leave to proceed in forma pauperis, he must pay the entire $350.00 filing fee in full REGARDLESS OF THE DISPOSITION OF THE DISTRICT COURT CASE (including dismissal).
