April 24, 2025 Opinion or Order Filing 10 ORDER. The clerk shall prepare a Notice of a Lawsuit and Request to Waive Service of a Summons (AO 398) form for each Defendant and send the forms to Plaintiff. Plaintiff shall have twenty-one (21) days from the date of this Order to sign the Notice of a Lawsuit and Request to Waive Service of a Summons (AO 398) forms and return them to the Court. Plaintiff shall have twenty-one (21) days from the date of this Order to submit six (6) services copies of the amended complaint, Doc. #9 . The service copies must be identical to the amended complaint filed with the Court. This case number should be written on the service copies. Alternatively, and within the same time, Plaintiff may submit $51.00 for the Court to make six (6) service copies of his 17-page amended complaint.
