Filing 2 ORDER re #1 Prisoner Consent. The clerk of the court shall forward to Plaintiff: (a) a prisoner civil rights complaint form; (b) a motion for leave to proceed in forma pauperis; and (c) a prisoner consent and financial certificate. This case number should be written on the forms. On or before October 21, 2019, Plaintiff shall file:(a) a notice of voluntary dismissal; or (b) a properly completed complaint form. If Plaintiff files a complaint, it must be typed or clearly written and submitted on the court-approved form. On or before October 21, 2019, Plaintiff shall either: (a) pay $400.00; or (b) submit a completed motion for leave to proceed in forma pauperis with the requisite supporting documents. Plaintiff's document entitled "To federal attorney" shall be returned to Plaintiff without filing. The clerk of court shall refer, on a deficiency order, any future document submitted for filing that fails to comply with court rules of procedure.
