Filing 4 ORDER denying without prejudice #2 Motion for Leave to Proceed in forma pauperis. The clerk of the court is directed to send Plaintiff: (a) a motion to proceed in forma pauperis for use by prisoners; and (b) a Prisoner Consent Form and Financial Certificate approved for use in the Northern District of Florida. This case number (5:20-cv-191-TKW/MJF) should be written on the forms. If Plaintiff wishes to proceed with this action, on or before 9/18/2020, Plaintiff shall either:(a) pay the $400.00 fee ($350 filing fee and $50 administrative fee) to the clerk of the court; or(b) submit a fully completed motion to proceed in forma pauperis with an attached Prisoner Consent Form and Financial Certificate signed by a prison official and a computer printout of the transactions in Plaintiffs inmate trust account during the six-month period of incarceration preceding the filing of this suit.
