Plaintiff is DIRECTED to complete said forms and return them within 20 days from the e ntry date of this Order. The Clerk is DIRECTED to submit this action if Plaintiff fails to comply. Upon receipt of forms, the Clerk is DIRECTED to prepare and mail a service waiver package for each Defendant. If Waiver of Service not returned in 35 days, the Clerk is DIRECTED to prepare a service package for service by the USM. This case SHALL proceed on a 4 month discovery track.
