OPINION AND ORDER granting Plaintiffs' Motion for Conditional Certification 29 and ordering that Notice shall be given to potential class members in the form attached to this Order. It is further ordered that Defendant shall, on or before May 25, 2017, provide to Plaintiffs a list of persons employed by Colibrium as non-exempt Sales Representatives at its Roswell, Georgia, facility who sold or renewed policies for Colorado HealthOp between September 1, 2014 and March 31, 2015. For these identified employees, Defendant shall provide their full name, job title, last known address and telephone number, date of employment, location of employment, and date of birth.
