Filing 4 ORDER: If Plaintiff does not choose to voluntarily dismiss, IT IS ORDERED that Plaintiff must, within THIRTY (30) DAYS of the entry date of this Order, either submit the full filing and administrative fees of $400.00, or execute and return a properly completed financial affidavit seeking leave to proceed in forma pauperis. The affidavit must include an executed authorization form allowing Plaintiffs custodian to withdraw funds from his inmate account on an incremental basis. Further, Plaintiff is DIRECTED that he must keep the Clerk of Court advised of his current address at all times while his case is pending. The Clerk of this Court is DIRECTED to forward Plaintiff the appropriate financial affidavit, together with a copy of this Order. The Clerk is further DIRECTED to RESUBMIT this action to the assigned Magistrate Judge upon receipt of the filing and administrative fees or affidavit or at the expiration of the aforementioned thirty-day time period.
