If Plaintiff does not choose to voluntarily dismiss, IT IS ORDERED that Plaintiff must, within THIRTY (30) DAYS of the entry date of this order, either submit the full filing and administrative fees of $400.00, or execute and return a financial affidavit seeking leave to proceed in forma pauperis. IT IS FURTHER ORDERED that Plaintiff must, within thirty (30) days of the entry date of this Order, submit an amendment in regard to prior lawsuits that he has filed in federal court. The Clerk of this Court is DIRECTED to forward Plaintiff a financial affidavit form. The Clerk is further DIRECTED to RESUBMIT this action to the undersigned Magistrate Judge upon the expiration of the aforementioned time period.
