ORDER directing the Clerk to convert this matter into a 28:2241 habeas corpus action and change the nature of suit code on the docket sheet accordingly. IT IS ORDERED that petitioner must, within THIRTY (30) DAYS of the date of this order, AMEND her petition by naming her immediate custodian as the proper party respondent and by completing in detail and submitting the habeas corpus form petition that will be provided to her by the Clerk of Court with this THIRTY (30) DAYS of the date of this order, pay the entire filing fee of $5.00 or seek leave to proceed in forma pauperis and return a financial affidavit along with a certified copy of her prison account statement. The Clerk SHALL stamp the civil action number on a copy of a 28 U.S.C. 2241 habeas corpus form (AO Form 242) and forward the form and the appropriate financial affidavit to petitioner, together with a copy of this Order. The Clerk SHALL also resubmit this action to the assigned Magistrate Judge upon receipt of the amended petition and filing fee or affidavit or at the expiration of the time period.
