ORDER: IT IS ORDERED that Plaintiff's motion for an extension of time [Doc. #3 ] is GRANTED, to the extent that Plaintiff shall have through September 15, 2021 in which to comply with the Court's July 16, 2021 Order, and that the motion otherwise is DENIED. Plaintiff is admonished that this action shall be subject to dismissal if he fails (1) to pay the fee or submit the required affidavit within the specified period of time or (2) to keep the Clerk of Court advised of his current address at all times while this action is pending. ORDER: IT IS ORDERED that Plaintiff must, within THIRTY (30) DAYS of the entry date of this Order, pay the $402.00 filing and administrative fees or properly execute and return the financial affidavit form provided by the Clerk of Court. The affidavit should include (1) an executed authorization form allowing Plaintiffs custodian to withdraw funds from his inmate account on an incremental basis and (2) a completed certificate, signed by an authorized institutional officer, stating the current balance in Plaintiff's inmate account and the average monthly balance and deposit in that account for the preceding six months. Plaintiff also shall include a certified copy of his inmate account statement for the preceding six months. The Clerk of this Court is DIRECTED to forward Plaintiff a financial affidavit form. The Clerk is further DIRECTED to RESUBMIT this action to the undersigned Magistrate Judge upon the expiration of the aforementioned time period.
