ORDER GRANTING Plaintiffs construed motion to proceed in forma pauperis, as amended Docs. #2 , #4 , is GRANTED. Kenneth Carson is ORDERED to pay the Initial fee of $34.00 and to make Monthly Payments as funds are deposited into Inmate Account., allowing Plaintiff to proceed IFP, however, Plaintiff is obligated to pay $350.00 as funds are deposited into Inmate account. The Clerk of the Court transmit a copy of this Order to the warden of the institution where Plaintiff is incarcerated; The warden of the institution, or his designee, shall collect the aforesaid monthly payments, including the initial $34.00 payment, from the Plaintiffs inmate account and remit such payments to the Clerk of the United States District Court for the Northern District of Georgia until the $350.00 filing fee is paid in full. Service of process shall not issue until further consideration of this complaint under 28 U.S.C. 1915A. The Clerk is DIRECTED to RESUBMIT this action to the undersigned Magistrate Judge upon the expiration of the aforementioned thirty-day time period. ORDER - If Plaintiff does not choose to voluntarily dismiss this action, IT IS ORDERED that Plaintiff must, within THIRTY (30) DAYS of the entry date of this Order, pay the $402.00 filing and administrative fees or properly execute and return the financial affidavit form provided by the Clerk of Court. During the pendency of his case, Plaintiff must keep the Clerk of Court advised of his correct address at all times. The Clerk of the Court is DIRECTED to forward Plaintiff a financial affidavit form.
