November 8, 2021 Opinion or Order Filing 3 ORDER. Petitioner must, within thirty (30) days of the entry date of this Order, pay the $5.00 filing fee or complete and return an updated in forma pauperis application and financial affidavit. The affidavit should include a completed certificate, signed by an authorized institutional officer, stating the current balance in Petitioners inmate account and the average monthly balance and deposit in that account for the preceding six months. Petitioner also must include an updated and certified copy of his inmate account statement. Petitioner is ADVISED that he must keep the Clerk of Court advised of his current and correct address at all times while his case is pending and FURTHER ADVISED that his action will be subject to dismissal under Local Rule 41.3 if he fails to keep the Clerk of Court advised of his correct address or if he fails to pay the filing fee or submit the required financial affidavit. The Clerk of the Court is DIRECTED to forward to Petitioner the appropriate financial affidavit form, together with a copy of this Order. The Clerk is FURTHER DIRECTED TO RESUBMIT this action to the assigned Magistrate Judge upon receipt of the filing fee or at the expiration of the above time period.
