ORDER GRANTING In Forma Pauperis, Directing Derek J. Burns to pay Initial fee of $ 21.00 within thirty days from the entry date of this Order and to make Monthly Payments as funds are deposited into Inmate Account, allowing Plaintiff to proceed IFP, however, Plaintiff is obligated to pay $350.00 as funds are deposited into Inmate account. The Warden or the designee shall collect the monthly payments and remit to Clerk until $350.00 is paid in full. Plaintiff is ADVISED that his action will be subject to dismissal under Local Rule 41.3 if he fails to pay the initial partial filing fee as ordered or if he fails to keep the Clerk of Court informed of his current address at all times while this action is pending. The Clerk is DIRECTED to RESUBMIT this action to the undersigned Magistrate Judge upon the expiration of the aforementioned thirty-day time period.
