ORDER: I DENY WITHOUT PREJUDICE Billy Joe Ken Mathiss application for permission to proceed in forma pauperis (IFP) #1 because he omitted the statutorily required certified copy of his inmate account statement, see [Doc. 1 at 1], did not answer the question asking how much money he has in cash or in a checking or savings account, see [id. at 2], and improperly attempted to limit his authorization for statutorily required future installment payments of the filing fee from his inmate account to $100, see [Doc. 2-1 at 5]. Accordingly, I ORDER Mathis within twenty-eight days of the entry date of this Order either (1) TO PAY $402 (if he is able to do so) or (B) TO FILE a complete IFP application, with all required information, signatures, certifications, and attachments. If Mathis does not wish to pursue this civil action and become liable to pay a filing fee (and, if applicable, administrative fees), then he must file a notice of voluntary dismissal pursuant to Federal Rule of Civil Procedure 41(a)(1)(A) within twenty-eight days of the entry date of this Order. I DIRECT the Clerk to enclose a copy of this Courts form IFP application for prisoners, pre-marked Second IFP Application / 1:23-CV-476-LMM-CMS. I caution Mathis that failure to promptly inform the Court of any change of address while this civil action is pending or to fully comply with any court order may result in dismissal.
