Williams, the prisoner-plaintiff who filed it, failed to disclose his prior litigation history. I DENY WITHOUT PREJUDICE Williams' application for permission to proceed in forma pauperis (IFP) because it makes self-contradictory claims and omits statutorily required signatures and attachments. If Williams objects to this Final Report and Recommendation, I ORDER him to include with his Objections either (1) payment of the $402 due in case initiation fees or (2) a complete IFP application with all required information, signatures, certifications, and attachments. I caution Williams that failure to comply with this order may constitute an additional and independent basis for dismissal of this civil action. See LR 41.3(A)(2), NDGa.
