Plaintiff also shall file an amended complaint and either pay the fee or submit a financial affidavit as previously ordered within TWENTY-ONE (21) DAYS of the date of this Order. Plaintiff must select only those claims that are logically related or arise out of the same transaction or occurrence to pursue in the instant action and may not incorporate by reference any portion of his original complaint into the amended complaint. IT IS FURTHER ORDERED that plaintiff must, within THIRTY (30) DAYS of the date of the entry date of this order, either submit the full filing and administrative fee of $402.00, or execute and return a financial affidavit seeking leave to proceed in forma pauperis. The Clerk of the Court is DIRECTED to stamp the civil action number on a standard 42 U.S.C. 1983 civil action form and forward the form and the appropriate financial affidavit to plaintiff, together with a copy of this Order.
