Filing 3 ORDER: Plaintiff is ORDERED to submit an amended complaint within THIRTY (30) DAYS of the entry date of this Order. Plaintiff must select only those claims that are logically related or arise out of the same transaction or occurrence to pursue in the instant action and may not incorporate by reference any portion of his original complaint into the amended complaint. IT IS FURTHER ORDERED that plaintiff must, also within THIRTY (30) DAYS of the date of this order, either submit the full filing and administrative fee of $402.00, or execute and return a financial affidavit seeking leave to proceed in forma pauperis that includes (1) a completed certificate signed by an authorized institutional officer regarding the current balance in his inmate account and (2) a copy of his inmate trust account balance for the six-month period preceding the filing of the instant action. Plaintiff is CAUTIONED that, even if he is allowed to proceed in forma pauperis, pursuant to 28 U.S.C. 1915(b)(1), he must nevertheless pay the full amount of the $350.00 filing fee from his prisoner account. The Clerk of the Court is DIRECTED to stamp the civil action number on a standard 42 U.S.C. 1983 civil action form and forward the form and the appropriate financial affidavit to plaintiff, together with a copy of this Order. The Clerk is further DIRECTED to resubmit this action to the assigned Magistrate Judge upon receipt of the amended complaint and full filing and administrative fee or affidavit or at the expiration of the time period noted above.
