Furthermore, Petitioner has not paid the $5.00 filing fee required for habeas petitions or submitted an application for leave to proceed in forma pauperis. If Petitioner wishes to proceed without prepayment of the filing fee, he must provide a completed application for leave to proceed in forma pauperis, including a financial affidavit and certified prison account statement. Accordingly, Petitioner is ORDERED to complete and submit the application within 30 days of the date this Order is entered. The Clerk is DIRECTED to forward a 28 U.S.C. 2254 petition form and an application for leave to proceed in forma pauperis stamped with the case number for this action to Petitioner together with a copy of this Order.
