February 16, 2024 Opinion or Order Filing 6 ORDER: Plaintiff is ORDERED to submit a second amended complaint within TWENTY- ONE (21) DAYS of the entry date of this order. IT IS FURTHER ORDERED that plaintiff must, also within TWENTY-ONE (21) DAYS of the date of this order, either submit the full filing and administrative fee of $405.00, or execute and return a financial affidavit seeking leave to proceed in forma pauperis that includes (1) a completed certificate signed by an authorized institutional officer regarding the current balance in his inmate account and (2) a copy of his inmate trust account balance for the six-month period preceding the filing of the instant action. The Clerk of the Court is DIRECTED to stamp the civil action number on a standard 42 U.S.C. 1983 civil action form and forward the form and the appropriate financial affidavit to plaintiff, together with a copy of this Order. The Clerk is further DIRECTED to mail a copy of this Order to the Fayette County Sheriffs Office and to resubmit this action to the assigned Magistrate Judge upon receipt of the second amended complaint and full filing and administrative fee or affidavit or at the expiration of the time period noted above.
