Filing 5 ORDER. The Sheriff of the Fulton County Jail is DIRECTED to complete that portion of the affidavit that requires a completed certificate signed by an authorized institutional officer regarding the current balance in Plaintiffs account and a certified copy of Plaintiffs inmate trust account balance for the last six months WITHIN THIRTY DAYS of the date of this Order. Plaintiff again is DIRECTED to amend the complaint and clarify whether he submitted the complaint on his own behalf as instructed in the February 6, 2024, Order. The Clerk of the Court is DIRECTED to forward the appropriate financial affidavit to Sheriff Labat and Plaintiff together with a copy of this Order. The Clerk is FURTHER DIRECTED to also send Plaintiff the appropriate forms for filing a 1983 complaint along with another copy of the February 6, 2024, Order. Signed by Magistrate Judge John K. IT IS FURTHER ORDERED that, should Plaintiff wish to proceed with this action, he must, within THIRTY DAYS of the date of this Order, AMEND the complaint on the proper forms. Plaintiff is FURTHER ORDERED to clarify, WITHIN THIRTY DAYS of the date of this Order, if he has submitted the complaint on his own behalf or if someone else has signed and submitted the complaint for him.
