ORDER GRANTING #5 In Forma Pauperis. The Warden of Plaintiffs place of incarceration or the Wardens designee SHALL COLLECT the monthly payments from Plaintiffs inmate account AND REMIT them to the Clerk of the United States District Court for the Northern District of Georgia until the $350.00 filing fee is paid in full, as verified by separate notice from the Clerk to the Warden. The Clerk SHALL TRANSMIT a copy of this Order to the Warden. Plaintiff will be required to pay the fee even if unsuccessful in this suit, but may avoid paying the fee by filing a notice voluntarily dismissing this action. Plaintiff is WARNED that the failure to inform the Court of each change of address while this civil action is pending may result in its dismissal, as provided in the Local Rules of this Court. ORDER: IT IS THEREFORE ORDERED that within 28 days Plaintiff must either submit the full filing and administrative fees of $400.00 or sign and return a financial affidavit seeking leave to proceed in forma pauperis (IFP) that includes Plaintiffs signed authorization allowing the custodian of Plaintiffs inmate account to withdraw funds from the account. Plaintiff need not re-file his inmate account statement. Plaintiff is therefore ORDERED to submit an amendment to his complaint, also within 28 days, to explain what steps he has taken to exhaust his administrative remedies and to name a proper Defendant. Plaintiff is DIRECTED to place the number of this case, 2:19-cv-0009-RWS-JCF, on the first page of his amendment. The Clerk is DIRECTED to forward to Plaintiff a prisoner financial affidavit and a 42 U.S.C. 1983 civil rights complaint form, together with a copy of this order, and to resubmit this matter to the Magistrate Judge upon receipt of the fees or affidavit, or when the 28-day period noted above expires.
