ORDER: IT IS THEREFORE ORDERED that Plaintiff must submit the full initial filing fee of $400.00, or a completed an executed financial affidavit seeking leave to proceed in forma pauperis on the proper forms, that includes: (1) an authorization allowing her custodian to withdraw funds from her inmate account; (2) a completed certificate signed by an authorized institutional officer regarding the current balance in her inmate account; and (3) a certified copy of her inmate trust account balance for the six-month period preceding the filing of the instant action certified by an authorized institutional officer. IT IS FURTHER ORDERED that Plaintiff must, within thirty (30) days of the date of this Order, AMEND the complaint in compliance with the Federal Rules of Civil Procedure. The Clerk is DIRECTED to forward the appropriate financial affidavit and the proper civil action form to Plaintiff and to SUBMIT this action to the undersigned when the thirty-day time frame set out above expires.
