Filing 4 ORDER GRANTING In Forma Pauperis and he shall be allowed to proceed without paying an initial partial filing fee. IT IS ORDERED that, within thirty (30) days of the entry date of this Order, Plaintiff must AMEND. Plaintiff is DIRECTED to (1) caption the amended complaint as an amendment to 2:19-CV-0155-SCJ-JCF and (2) recast his complaint and proceed either on his claim against Officer Crawford or his claim against Nurse Armenti. Plaintiff is DIRECTED that he must keep the Clerk of Court advised of his current address at all times while his case is pending. Plaintiff is CAUTIONED that his action will be subject to dismissal if he does not comply with the Courts Order to amend or comply with the Court's direction to keep the Clerk of Court advised of his current address at all times while his case is pending. IT IS FURTHER ORDERED that: The Clerk of the Court transmit a copy of this Order to the warden of the institution where Plaintiff is incarcerated; and The warden of the institution, or his designee, shall collect the aforesaid monthly payments from Plaintiff's inmate account and remit such payments to the Clerk of the United States District Court for the Northern District of Georgia until the $350.00 filing fee is paid in full. The Clerk is DIRECTED to forward Plaintiff two 42 U.S.C. 1983 civil action forms. The Clerk is DIRECTED to RESUBMIT this action to the undersigned Magistrate Judge upon the expiration of the aforementioned thirty-day time period. Signed by Magistrate Judge J.
